DATA PRIVACY / CONSENT

The Philippine Normal University recognizes its responsibility under the Republic Act No. 10173, also known as the Data Privacy Act of 2012. The personal data obtained from this Personal Inventory Record is stored in a confidential file accessible only to authorized persons.

The information collected shall be used for student welfare, research purposes with protection for individual confidentiality, contacting students, and, in emergency cases, their authorized guardians and / or emergency contact person. PNU shall not disclose the student’s personal information without his/her consent.

CONSENT:

I have read the Data Privacy Statement and express my consent for Philippine Normal University to collect, record, organize, update, use for student welfare and research purposes with protection for my identity, contact me, notify my authorized guardian / emergency contact person for emergency purposes, and destroy / delete the file based on the institution’s policies. I also agree to update my contact numbers as well as the contact numbers of my authorized guardian/ emergency contact person.

I also certify that the information contained here is accurate to the best of my ability.
